A 2023 analysis of the National Electronic Injury Surveillance System estimated more than 250,000 stair-related upper-extremity fractures among Americans 65 and older across a single decade, with hospitalization rates climbing throughout the study period. Numbers that big don’t come from clumsy people. They trace back to decisions made years earlier on a drafting screen: a tread depth, a handrail profile, a paint sheen, a lighting plan. The person who eventually falls never meets the designer, but the design met them first.

Fall prevention gets talked about like a facilities problem or a caregiver problem. It’s a design problem, and the field is thick with confident assumptions that don’t survive contact with real bodies on real stairs. 

Here are the ones worth correcting.

Code Compliance Is a Floor, Not a Ceiling

The most common myth in practice is that a code-compliant stair, ramp, or bathroom is a safe one. Codes set minimums. They tell you what you can’t get away with, and they say very little about what will work for the people who actually use the space.

The U.S. Access Board’s stairways guide is a good example. It spells out riser and tread dimensions, handrail extensions, and opening limits in nosings, and it’s worth reading closely.

What it stops short of is telling you what makes a stair intuitive to descend in low light, in bifocals, with a laundry basket in your arms. Two stairs can both pass inspection and behave completely differently under the foot. The designer’s work starts where the code stops.

Falls Are Not a Senior-Only Problem

Older adults carry the worst outcomes, so they dominate the conversation. One assumption slips in behind that: if the building isn’t senior housing, fall prevention is somebody else’s concern.

Toddlers fall on the same stair nosings. Delivery workers slip on the same polished lobby tile in the rain. Restaurant staff misjudge the same unlit half-step between the dining room and the kitchen.

The specific injuries differ, but the design defects don’t discriminate. A single hazardous transition in a public space will find every demographic that walks through it.

Slip Resistance Is a Specification, Not a Vibe

Designers often pick flooring based on how it looks in a sample, how it photographs, and what the rep says. Slip resistance ends up as a footnote in the finish schedule, if it appears at all.

There are published, testable numbers for this. Industry guidance points to a static coefficient of friction of at least 0.5 as a floor for walking surfaces, with higher targets for wet areas and ramps.

The number matters more than the finish name. A honed stone that scores well beats a textured-looking tile that doesn’t, and neither one is safe if it’s specified without knowing the value. Ask the manufacturer for the tested figure, wet and dry, and keep it in the record.

Handrails Aren’t Decoration You Grab in an Emergency

The handrail-as-accent-piece habit runs deep. Square profiles, oversized timber, wall-hugging brackets, breaks at landings. It photographs beautifully, and it fails the hand that’s already sliding.

A handrail has to be graspable in a closing fist, at speed, by someone who isn’t looking at it. That means specific things you can measure:

  • Diameter. A round rail in the 1¼” to 2″ range gives a full-hand grip, which wider decorative profiles can’t match.
  • Clearance. Roughly 1½ inches between rail and wall lets fingers wrap without scraping.
  • Continuity. The rail should carry through landings and extend past the top and bottom risers, not stop where the wall ends.
  • Both sides. Single-side handrails assume the user’s dominant side is available, and in a real fall it often isn’t.

Lighting Is a Fall-Prevention System

Lighting gets treated as an ambiance decision, which is how you end up with dim hallways, glossy treads under a single downlight, and step edges that visually disappear into the tread field.

The eye needs contrast at edges and consistent levels through transitions. Pools of bright light next to pools of dark force the pupil to keep adjusting, and that adjustment is where people miss a step. Contrasting nosings, evenly distributed light on every tread, and switches at both ends of a stair are baseline for any surface people are asked to descend, not an upgrade.

Nobody Sues the Architect First, and That’s Not the Point

When someone falls in a building, the property owner is usually the first name on the paperwork. Designers can read that and assume the risk sits somewhere else. It doesn’t, at least not fully.

Premises liability cases routinely pull in the specifications, the finish selections, and the drawings, and the attorneys who handle these cases will look hard at whether the fall was a one-off or a predictable outcome of choices made on paper.

The clean version: if a hazard is foreseeable from the design, someone will eventually make that argument. The design record will either support the decisions or leave them exposed. That’s a good reason, independent of liability, to write down the slip-resistance numbers, keep the daylighting studies, note the handrail rationale, and specify assuming the least-abled user will one day be on the stair.
